Therapy for Young Children with Tantrums, Defiance, and Aggression in Los Angeles

When a young child has frequent tantrums, refuses directions, becomes aggressive, or turns everyday routines into repeated power struggles, family life can feel exhausting. Many parents feel worried, frustrated, and unsure what to do next. They may have tried different approaches, but nothing seems to work consistently. 

At Jacobs Psychological Services, I provide Parent-Child Interaction Therapy (PCIT) for young children with tantrums, defiance, aggression, and ongoing parent-child conflict. PCIT is an evidence-based treatment for children ages 2 to 7 that helps parents strengthen their relationship with their child while learning practical skills to improve behavior, reduce daily struggles, and set effective limits. Are Tantrums and Noncompliance Normal?

Yes. Some tantrums, noncompliance, and limit-testing are a normal part of early childhood, especially during the preschool years. Young children are still learning how to manage frustration, tolerate limits, shift between activities, and communicate their needs.

At the same time, the way adults respond to these behaviors can make them easier or harder for a child to outgrow. Some children, even within the range of typical development, have temperaments that make certain responses from adults more likely. A child who is especially intense, persistent, emotionally reactive, or hard to soothe may draw caregivers into patterns such as repeating directions, negotiating, giving in, escalating, or becoming inconsistent.

This does not mean the parent is at fault. It means that child behavior and parent responses influence each other over time. Families can become stuck in patterns that unintentionally strengthen tantrums, defiance, or noncompliance. PCIT helps parents change these patterns by strengthening the parent-child relationship while also teaching clear limits, consistency, and calm follow-through. 

When Might Therapy Help?

Many young children with tantrums or noncompliance do not need testing or intensive treatment. Therapy may help when these behaviors become frequent, intense, prolonged, or disruptive to daily life, or when family routines are increasingly dominated by conflict.

PCIT may be a good fit when a young child is struggling with:

  • severe tantrums 
  • defiance or refusal to follow directions 
  • hitting, kicking, biting, or throwing objects 
  • aggression toward parents, siblings, peers, or property 
  • prolonged meltdowns or difficulty calming down 
  • daily power struggles around routines, transitions, or limits 
  • increasing stress in the parent-child relationship 

In many cases, the issue is not that something is seriously wrong with the child. Rather, the family needs a more effective and consistent way of responding to behavior. Because PCIT focuses on both child behavior and caregiver response patterns, it can be especially helpful when families feel stuck in cycles that are no longer working. 

When Might a Broader Evaluation Be Needed?

Although tantrums, noncompliance, and limit-testing are often a normal part of early childhood and many children outgrow them, in some cases these behaviors may reflect a broader developmental picture. Most young children with tantrums or noncompliance do not have a neurodevelopmental disorder such as autism spectrum disorder (ASD) or attention-deficit/hyperactivity disorder (ADHD). However, a more comprehensive psychological or developmental evaluation may be important when these behaviors occur alongside broader concerns about development, communication, social interaction, attention, learning, or behavior regulation.

An evaluation may be appropriate when there are concerns such as:

  • significant delays in language or communication 
  • reduced social engagement or limited reciprocal interaction 
  • difficulty with back-and-forth communication 
  • unusual sensory sensitivities or sensory-seeking behaviors 
  • repetitive behaviors or highly restricted interests 
  • marked rigidity or distress with changes in routine 
  • developmental delays across multiple areas 
  • concerns about attention, impulse control, learning, or daily functioning 

In these situations, PCIT may still be helpful, but it may not be sufficient on its own. A comprehensive evaluation can help clarify whether autism spectrum disorder, ADHD, language delays, cognitive differences, or another neurodevelopmental condition may be contributing to the child’s presentation. This helps guide treatment planning and identify the interventions and supports that may best meet the child’s needs. The goal is not to pathologize typical childhood behavior, but also not to overlook signs that a child may need a broader evaluation.

PCIT is an active, practical treatment for children ages 2 to 7 with tantrums, aggression, defiance, noncompliance, and parent-child conflict. Parents receive live coaching while interacting with their child, helping them learn how to respond in the moment and use those same skills at home.
